WELLNESS CLUB MEMBERS: Please fill out the form below to contact us with any questions or feedback. Individuals, staff and members using wellness club benefits, please use your assigned site for your group's sponsored services or 'members only' benefits. Any questions or assistance needed, please contact us at the phone or email below.
INDIVIDUAL/PATIENT REFERRALS: If you are patient referred by your
physician for a Wellness Solutions Complimentary Consult, please follow the
directions to the site listed on your referral card to set up your complimentary phone
or email consult. Be sure and include your referring physician's name and the referral
code on the consult card they gave you. We look forward to initiating a secure, confidential email or phone consultation to review your options and your physician's recommendations.*
Please have your Dr. or referring health professional complete the appropriate referral forms below and fax them to our secure fax: 281-974-2827. Be sure the form is completely filled out and signed by you along with your Dr. or health/professional making the referral.
If you are initiating service for your self and plan on using your medical insurance or are on medications, we will discuss your medical/medication management at our first meeting. Please fill out the "New Client Info" form (at bottom of page) & 'SELF' Referral form and fax them to our secure fax: 281-974-2827 when you schedule your appointment.
*If you are experiencing an emotional crisis or emergency, please call your physician, 9-1-1 or
go to your nearest Emergency Room for assistance. We make every effort to respond to inquiries/requests within 24-72 hrs. This site is not a replacement for emergency services if you
are in crisis.
